This online portal is strictly for use by existing, active patients of the physicians at Lancaster Medical Family Practice. If you are not a registered patient at our clinic, your request will not be processed.

Our Patient Portal allows you to easily submit forms, update your personal information, and request specific medical documents without needing to call the clinic or wait on hold. To protect your privacy, all submissions are processed through a fully encrypted, secure medical network.

What can I do through the portal?

You can use this system to seamlessly submit the following requests:

Request a sick day(s) note
Request a note for massage therapy, orthotics, psychotherapy, osteopathy, etc.
Request a FIT kit for colon cancer screening
Update your Health Card or Contact Information
Submit a medical form that needs to be completed by your physician
Submit vaccination information
Request online completion of the Federal Disability Tax Credit

Uninsured Services

Please be aware that certain items, such as sick notes and form completions, are not covered by OHIP. You may be prompted to pay an uninsured service fee for these requests.

Not for Emergencies

This portal is monitored during regular business hours only. Do not use this system for urgent medical issues. If you are experiencing an emergency, please call 911 or visit the nearest emergency department.
